Vital Balance Polices
Privacy Policy Effective Date: 1st January 2026
Health Intake Forms - Before Your Appointment
To make sure your treatment is safe, effective, and truly tailored to you, we ask all clients to complete a health intake form before their appointment.
New clients
If you’re visiting us for the first time, you’ll need to fill out a health intake form before your treatment begins. This gives us a clear understanding of your current health, so we can support your body in the best possible way. Keeping your information up to date
We ask that your health information is updated every two years, or sooner if anything changes. If you’ve had any new injuries, diagnoses, or changes in how your body is feeling, please let us know — even if it seems small. It all helps us take better care of you.
Pregnancy
If you are pregnant, we’ll ask you to complete a specific pregnancy form at the start of your treatment journey with us. This ensures your care is appropriate for each stage of pregnancy.
Cancer care / oncology treatments
If you have a current or recent cancer diagnosis, we’ll provide a specialised oncology form prior to treatment. This allows us to tailor your session safely and respectfully to where you’re at.
A gentle note
We’re here to support you, not to overwhelm you with paperwork. These forms simply help us understand what your body needs, so you can relax knowing you’re in safe hands.
